Zoom Event Inquiry & Planner Meeting Request for UC Department

Thank you for your interest in using Student Center & Event Services (SCES) to host your upcoming Zoom Webinar.

This form will gather the information necessary for SCES to evaluate whether resources are available for this online event. If resources are available, you will be contacted about next steps. The planner will walk you through the event planning process and provide you with key information.

This form should be completed by a full-time UC employee authorized to sign agreements and commit funds via KFS/interdepartmental recharge.

NOTE: Event lead times will vary based on the size and scope of an event.

Co-Sponsor Names

Co-sponsorship is defined as events that meet one or more of the following criteria:

  1. More than one organization is involved in the planning process–which includes verbal and written instructions–and modifications to existing event contracts and/or
  2. Funding for the event comes from organizations or individuals other than the primary sponsor and/or
  3. Publicity of the event indicates sponsorship involvement including more than the primary sponsor or multiple sponsors.

Basic Webinar Information

Billing

All event related charges will be billed to you via KFS or Interdepartmental recharge. Please provide the KFS or interdepartmental recharge number.

Note: Requests to bill more than one account are considered on a case-by-case basis. When such requests can be accommodated, an administrative fee of $50 per additional payment account is applied to the event bill.
